  • First, Second, Third BAMS Mark sheets (Final Part-I & Part-II)
  • Whether Internship was started on or before 1st November 20-------.(Previous Year)
  • Whether Internship is being completed within 31st December 20--.(Previous Year).
  • BAMS Degree/Provisional certificate
  • Permanent/provisional Registration Certificate issued by CCIM or State Medical Counsel . Provision Registration Certificate is acceptable only in cases where candidate is undergoing internship land likely to complete the same on or before 31st December 20--.(Previous Year).
